Methods We collected data for smoking status,...Objective This study explored the role of airway hyperresponsiveness in the development of chronic obstructive pulmonary disease (COPD) in severe smokers. Methods We collected data for smoking status, forced expiratory volume in one second (FEV1) , forced vital capacity (FVC) , and metha-choline provocation test of airway responsiveness in 456 severe smokers (more than 15 pack-years) without chronic respiratory symptoms and any severe cardiac and cerebral vascular diseases in the year 1996. 259 subjects were reinvestigated on lung function and occurrences of COPD in the year 2002. Results After adjustment for age, sex, body height, smoking pack-years, cigarette types ( filter, non filter, both) , stopping smoking, and FEV1 at baseline, compared with normal responders, airway hyperresponsiveness ( AHR, PC20 FEV18 mg/ml) were independently associated with an accelerated decline in FEV1(β= 19. 2 ml/yr, P = 0.003), FVC(β=20.6 ml/yr,P = 0.012) and FEV1/FVC(β= 0. 4%/yr, P=0.008) from 1996 to 2002. When compared to normal responders, AHR subjects without initial COPD were more likely to acquire future COPD (odds ratio; 5.6; 95% conference interval: 1. 1-28. 2) after adjusted for age, sex, height, pack-years of cigarettes smoked, filter cigarettes, stopping smoking, and baseline FEV1. Conclusion Airway hyperresponsiveness plays important role in COPD induced by smoking. Airway responsiveness test will be available to find out vulnerable smokers in earlier stage of COPD.Read More
