Title: Application of albumin combined with β_2-microglobulin and cysteineproteinase in the early diagnosis of diabetic nephmpathy
Abstract: Objective To investigate the application of albumin(mAlb) combined with β2-microglobulin(β2-MG) and cysteineproteinase(Cys-C) in the early diagnosis of diabetic nephmpathy(DN). Methods The DN patients were divided into the microalbuminuria urinary protein group(group A, n=35), the macroalbuminuria group(group B, n=35) and healthy control group(the control group, n=36), according to the removal rate of urinary protein(UAER). The levels of mAlb, β2-MG and Cys-C between the three groups were compared. The correlation between the levels of mAlb, β2-MG, Cys-C and the risk of DN as well as the detection accuracy of mAlb associated β2-MG and Cys-C were analyzed. Results The levels of mAlb, β2-MG and Cys-C in group A and group B were all significantly higher than those in the control group(P0.05). The levels of mAlb, β2-MG and Cys-C in group B were significantly higher than that in group A(P0.05). The correlation coefficient between mAlb, β2-MG, Cys-C levels and the DN risk danger were 2.92, 2.75 and 2.60, all showing significantly positive correlation(P0.05). The detection sensitivity, specificity and diagnostic accordance rate of combined detection of mAlb, β2-MG and Cys-C reached 94.45%,94.64% and 93.17%, respectively, all significantly higher than the value of separate detection and combined detection of two indexes(P0.05). The false positive rate and false negative rate was only 5.36% and 5.55%, significantly lower than the detection value of separate detection and combined detection of two indexes(P0.05). Conclusion There are significantly correlations between mAlb, β2-MG, Cys-C and the renal injury. The combined detection of mAlb,Cys-C and β2-MG has higher accuracy on early diagnosis of diabetic nephropathy and could be used as a method of early clinical diagnosis on DN.
