Title: Value of CT-guided intra-lesion injection of contrast media in suspected hepatic abscess
Abstract: Objective To investigate the value of CT-guided intra-lesion injection of contrast media in hepatic abscess with an indeterminate diagnosis before percutaneous drainage.Methods 21 patients with suspected hepatic abscesses without pus on needle aspiration were evaluated by CT-guided percutaneous intra-lesion injections of contrast media and biopsies using coaxial needles.Drainage was performed only when intra-lesion injection of contrast media supported the diagnosis of hepatic abscess.The CT appearance of intra-lesion injection of contrast media was correlated with pathological findings and the effect of drainage.Results Abscess cavities with honeycomb-like contrast filling were confirmed histologically in 14 patients.The abscesses were decompressed by drainage with symptomatic relief.Of 7 patients with contrast accumulating at the needle tips and no abscess cavities,pathological diagnosis included hepatocellular carcinoma(4),cholangiocarcinoma(l),and metastases(2).Conclusions Intra-lesion injection of contrast media is helpful in the diagnosis of hepatic abscess when needle aspiration does not yield typical pus.
