Title: Immediate and long-term effects of bisoprolol on the clinical characteristics and hemodynamics in patients with the chronic heart failure caused by valvular heart disease and atrial fibrillation
Abstract: Objective To investigate immediate and long term effects of bisoprolol on the clinical characteristics and hemodynamics in patients with chronic heart failure(CHF) caused by valvular heart disease and atrial fibrillation(AF) with rapid ventricular reaction.Methods Fifty one valvular heart disease patients with CHF(NYHAⅢor Ⅳ),heart/chest ratio 65% and AF with ventricular rate 100bpm were randomized to treated group(n=27) or placebo group( n =24).All patients received diuretic,digitalis,ACE inhibitor and nitrate by valvular lesions and blood pressure level.The treated group was also given bisoprolol.The immediate and long term (6 months) clinical and hemodynamic variables were compared in 2 groups.Results Compared with placebo group,the treated group had a shorter stay length,slower ventricular rate at 24h in admission.LVEF and 6 minute walking test improved in the treated group at discharge.After 6 months of treatment with bisoprolol.LVEF or NYHA and left atrial volume in treated group had significantly improved ( P 0.05).A trend towards a obvious reduction in combined end point was also observed in treated group.The increase of LVEF in treated group may be caused by a reduction in ventricular rate and left atrial volume.Conclusion Bisoprolol could improve the immediate and long term life quality and hemodynamics in patients CHF caused by valvular heart disease and AF with rapid ventricular reaction.
