Abstract:Objective:Investigate the impact of protamine counteract to heparin on different pH after cardiac surgery.Methods:We performed operation to 352 cardiac patients under cardiopulmonary bypass during fro...Objective:Investigate the impact of protamine counteract to heparin on different pH after cardiac surgery.Methods:We performed operation to 352 cardiac patients under cardiopulmonary bypass during from Oct.2009 to May 2010.After the implementation of the anesthesia,we detected normal activated clotting time.And then we injected heparin from the central vein by 2.5 mg/kg when we open the pericardium.At the end of cardiopulmonary bypass,we injected protamine at 1.5 times dose of heparin in the aortic root.We meas-ured antagonismed normal activated clotting time,and depended on which we added protamine.We divided them into three groups according to pH at the end of cardiopulmonary bypass.We observed each group of the ratio of heparin and protamine,ACT values after antagonism.We observed three groups of patients drainage at 6 h,12 h,24 h,and the total postoperative drainage.Results:In the acidic environment,more protamine dose and postoperative drainage are needed than the other two groups,There was remarkable difference(P 0.05).In the neutral and alkaline environment,the mean ratio of protamine and heparin is 1.63∶ 1.when pH is less than 7.31 or base excess -6.The amount of protamine will significantly increased.The ratio of protamine and heparin is 2.33∶ 1.Conclusion:There is great influence on protamine counteract to heparin at different pH.In the acid environment the amount of protamine and the postoperative chest drainage will increase,We recom-mend to increase the amount of protamine and correct the acid-base balance if pH 7.31 or BE -6 during the end of CPB.Read More
