Title: Timing of Start of Controlled Ovarian Hyperstimulation after Pituitary Down-regulation by Depot GnRH Agonist
Abstract: Objective:This study observed the influence on hormone,oocytes,embryos and clinical outcome when delayed the starting time of controlled ovarian hyperstimulation(COH) after a half-dose depot GnRHa.Methods:According to the type of GnRHa and different starting time of COH,a total 207 cycles were divided into 3 groups:control group(which performed daily low dose GnRHa during the mid-luteal phase of their menstrual cycles and reduced to 0.05 mg at day 3 of the menstrual cycle when begin COH),conventional group(in which pituitary desensitization was obtained with a half-dose depot GnRHa in the mid-luteal phase and the beginning of COH was at day 3) and delayed group(which has same use of GnRHa with conventional group,but the beginning of COH was delayed to day 7).Results:The cancellation rate in conventional group was the highest(P0.01).On the beginning of COH,serum E_2 and LH level in delayed and controlled group were significantly higher than conventional group(P0.01).On the day when HCG,serum E_2 level in control group is the highest,in conventional group is the lowest,which had statistical significance with each other in these three groups(P0.05).LH level in delayed and control group was higher than in conventional group(P0.01).Concerning the clinical efficacy and outcome,the numbers of Gn ampoules and period for stimulation were lower in delayed group than in conventional group;the numbers of retrieved oocytes and fertilized oocytes,clinical pregnancy rate and embryo implantation rate in delayed and control groups were significantly higher than in conventional group(P0.01).In ICSI cycles,the number of retrieved oocytes and metaphase Ⅱ oocytes in delayed and control group were higher than conventional group.Conclusions:A half-dose depot GnRHa may produce over suppression to pituitary gland in IVF,appropriate delay of starting time of COH can make ovarian stimulation period and ampoules of Gn decrease,retrieved more oocytes with high quality,so we could get a higher number of good quality embryos with a good chance of implantation that is very important for PR in IVF.
