Title: The results of tuberculosis chemotherapy in chest clinic practice
Abstract: One hundred and forty-three patients, most of whom had started triple chemotherapy in hospital, were treated as chest clinic out-patients with daily self-administered isoniazid and PAS. Drug-taking was carefully monitored. At assessment at one year, among 117 initially sputum-positive patients, 105 were sputum-negative and only two were still sputum-positive. Of the remaining 10 patients, 1 had died with active tuberculosis, 7 had died sputum-negative, and 2 had defaulted. All 94 patients available for assessment at three years showed no clinical or radiographic evidence of relapse. About a half were still producing sputum, but all cultures were negative. Toxic effects caused the offending drugs to be abandoned in 10%. Allergic reactions occurred in 13%. Eight percent of the patients did not complete 18 months treatment because they were unco-operative. About 20 % of patients were judged to be taking the drugs irregularly. It is concluded that carefully monitored self-administered daily isoniazid and PAS gave excellent results. Fully supervised intermittent chemotherapy is used only for selected patients. En conclusion, un traitement soigneusement controlé administre par le malade lui meme, et comprenant quotidiennement isoniazide et PAS a donné d'excellents résultats. Une chimiotherapie intermittente, complétement supervisée est utilisée seulement pour une selection de malades. De los 10 restantes, 1 murió con tuberculosis activa, 7 murieron con baciloscopia negativa y 2 abandonaron. Los 94 pacientes que pudieron controlarse a los 3 años seguían negativos. Las manifestaciones tóxicas provocaron abandonos en el 10 %. Hubo reaciones alérgicas en el 13 %. El 8 % de los pacientes no completaron los 18 meses de tratamiento por falta de colaboración. Se considera que el 20 % de los pacientes tomó las drogas en forma irregular. Se concluye que el suministro diario, bien supervisado, de isoniazida y PAS dió excelentes resultados. Un esquema intermitente supervisado se usó solamente para pacientes seleccionados. Von den verbleibenden zehn Patienten starb einer mit aktiver Tuberkulose, bei sieben war das Sputum vor dem Tode negativ geworden und zwei gingen aus der Beobachtung verloren. Alle 94 nach drei Jahren noch erreichbaren Patienten waren in der Kultur negativ. Toxische Erscheinungen zwangen bei 10% zum Therapieabbruch. Allergische Erscheinungen traten bei 13 % auf. Acht Prozent der Patienten führten wegen unkooperativen Verhaltens die Behandlung nicht fiber die vorgesehenen 18 Monate durch. Bei rund 20% der Patienten wird unregelmäßige Tabletteneinnahme unterstellt. Es wird gefolgert,daß sorgfältig überwachte tägliche Einnahme von Isoniazid und PAS ausgezeichnete Resultate erbrachte. Voll kontrollierte intermittierende Chemotherapie wird nur fur wenige Patienten benötigt.
