Title: Prospective study of Comparing Axillary Lymphnodes using Axillary Reverse Mapping during Modified Radical Mastectomy
Abstract: BACKGROUND AND OBJECTIVES:
The aim of this study is to identify and preserve the nodes draining arm from those draining breast using reverse axillary mapping technique in patients undergoing modified radical mastectomy in preventing lymphedema complication.
METHOD:
A total of 60 patients of Carcinoma Breast who underwent Modified Radical Mastectomy in General surgery department, Government Rajaji Hospital, Madurai during the period from September 2018 to September 2019, were included in this prospective study, and randomized into two groups with 30 patients undergone MRM, with axillary dissection using axillary reverse mapping (Group A) and 30 patients undergone MRM with axillary dissection without axillary reverse mapping (Group B). Patients were evaluated for increase in midarm circumference POD 14, 30 and 60 and lymphedema complication.
RESULT:
There was no significant difference in midarm circumference in both groups on POD 14 (p>0.05).
There was significant increase in midarm circumference in 3 patients in Group B compared to 1 patient in Group A on POD 30 and 4 patients in Group B compared to 1 patient in Group A on POD 60 with both p values <0.05.
1 patient developed lymphedema in Group A vs 4 in Group B. p value was <0.05.
CONCLUSION:
This study demonstrated that the axillary dissection using axillary reverse mapping significantly decreases the incidence of post-operative lymphedema.
