Title: A Case of Takotsubo Cardiomyopathy Induced by Accidental Subcutaneous Epinephrine Overdose Injection
Abstract: A 41-year-old woman had a nose surgery in another hospital. During operation she was accidentally given 1 mg subcutaneous epinephrine instead of 0.03 mg. Ventricular tachycardia subsequently developed and lasted 30 seconds. After the operation ECG was not changed but echocardiography revealed bulging out of the middle to apex of the heart with reduced ejection fraction (EF) to 36%The patient was immediately transferred to our hospital. There were no ECG change and no symptom. In the course of epinephrine overdose, we suspected takotsubo cardiomyopathy and observed her in intensive care unit. To prevent left ventricular thrombus formation, we used continuous infusion of heparin. On the next day ECG changed to negative T and prolonged QT interval. We observed improved EF (52% on day 4 and 57% on day 7) and normalized QT interval and she discharged. After 1 month T wave inversion was still remaining in the outpatient clinic. We report this case with some literature review.
