Title: Clinical observation of therapeutic effect of telmisartan on early type 2 diabetic nephropathy
Abstract: Objective To investigate the therapeutic effect of angiotensin Ⅱ receptor antagonist telmistartan on patients with early type 2 diabetic nephropathy(DN).Methods Sixty-four diabetic patients with urine albumin excretion rate(UAER) between 20 and 200 microgram per minute and normal blood pressure were divided into treatment group and control group. The former group received telmistartan 80 milligram once daily while the latter group received the same dosage of placebo on the basis of blood glucose control.Urine protein, blood pressure, renal function, serum potassium and glycosylated hemoglobin were measured before and after the treatment. Results The levels of 24-hour urine protein excretion,β2-microglobulin were significantly decreased after treatment in treatment group(P0.05),significantly lower than those in control group in the meanwhile. There weren′t significant differences in blood pressure, renal function indices, serum potassium and glycosylated hemoglobin between the two groups before and after the treatment(P0.05).Conclusion Telmistartan contributes to lessening of urine microalbumin excretion rate and β2-microglobulin in patients with early type 2 diabetic nephropathy with normal blood pressure. It posseses special kidney protection effect independent of its antihypertensive effect.
