Title: Application of MR diffusion tensor imaging for evaluation of cerebral glioma grading
Abstract: Objective To grade cerebral glioma by testing fractional anisotropy(FA) and mean diffusivity(MD) of diffusion-tensor MR imaging.Methods MR DTI was performed in 28 patients which histologically proved as cerebral glioma.Diffusion sensitive coefficient(b value=0s/mm2 and 800s/mm2).The value of the tumor essence and the ROI area of contralateral normal tissue were surveyed separately based on the FA map and the MD map.Comparisons of mean diffusivity(MD) and fractional anisotropy(FA) were done between tumor parenchymas regions and contralateral normal tissues and between various ranks.Results The MD values of grade Ⅰ~Ⅱ glioma parenchymas regions [(1.561±0.363) ×10-3mm2/s] were significantly higher than those of gradeⅢ [(1.148±0.215)×10-3mm2/s](P0.01)or gradeⅣglioma [(1.172±0.263)×10-3mm2/s](P0.01),but the difference between gradeⅢ and grade Ⅳ was not significant.The FA values of grade Ⅰ~Ⅱ glioma parenchymas regions(0.157±0.016) were significantly lower than those of gradeⅢ(0.233 ±0.031) or grade Ⅳ glioma(0.237±0.033)(P0.001),respectively.The FA values between grade Ⅲ and grade Ⅳ were not different significantly,but there were significant difference in MD and FA between the ROIs of gliomas parenchymas and contralateral normal white matter.The FA maps could demonstrate the relations of tumor and the white matter fiber clearly.Conclusion MD and FA,two parameters of DTI,have the clinical value regarding diagnosis of the cerebral glioma gradation.
