Title: Clinical and Pathological Analysis of IgA Nephropathy with Manifestations of Nephritic Syndrome in Children
Abstract: Objective:To analyse clinical and pathologic features of immunoglobulin A nephropathy (IgAN) with manifestation of nephritic syndrome in children.Methods:A retrospective review of 98 cases of primary IgAN within 9 years(1995to2004)were conducted based on renal biopsy,to summarize clinical manifestations and pathological features of IgAN with manifestation of nephritic syndrome.Results:According to WHO pathological changes of 2 cases were grade Ⅰ(9.1%),1 cases with simple nephropathy,1 cases with nephritis nephropathy;5 cases were grade Ⅱ(22.7%),1 cases with simple nephropathy,4 cases with nephritis nephropathy;9 cases were grade Ⅲ(40.9%),6 cases were grade Ⅳ(27.3%),all cases with nephritis nephropathy;Immunofluorescence typing;IgA 5 cases(22.7%),IgA+IgG 4 cases(18.2%),IgA+IgM 9 cases (40.9%),IgA+IgG+Ig 4 cases(%),IgA+IgM was frequently found;Tubulointerstitial lesions (TIL) grading:11 cases were grade Ⅰ (50.0%),7 cases were grade Ⅱ(31.8%),4 cases were grade Ⅲ(18.2%),grading Ⅳ was not found.Conclusion:The clinical representation and tissue damage of IgA nephropathy presenting with nephritic syndrome are serious,grades Ⅲ and Ⅳ pathologically and type IgA+IgM immunologically are common,grades Ⅰand Ⅱ in TIL are frequently found.
