Title: Expression of apoptosis inhibitor gene survivin in gastric cancer and it's clinical implication
Abstract: Objective To study the expression of survivin, apoptosis inhibitor gene, in gastric cancer and investigate its clinical-pathological implication. Methods We detected the expressions of survivin in 65 cases of gastric cancer and 20 cases of benign gastric ulcers by immunohistochemistry (Envision's method) and investigated its relation with the clinic-pathological parameters of gastric cancer. Results The overall positive rate of survivin in 65 cases of gastric cancer was 53.85%(35/65). No survivin was found in 20 cases of benign gastric ulcer. The positive rate of survivin in stage Ⅰ, Ⅱ, Ⅲ and Ⅳ gastric cancer was2/8, 26.32%(15/19), 74.07%(20/27) and 72.73(8/11), respectively. And the positive rate in stage both Ⅰand Ⅱ gastric cancer was 25.93%(7/27) and in stage both Ⅲ and Ⅳ 73.68%(28/38) ( P 0.01). The positive rate in gastric cancer with and without local lymph node metastasis was 62.86%(22/35) versus 43.33% (13/30) ( P 0.05). The positive rate in gastric cancer with and without distant metastasis was 7/9 versus 50.00% (28/56)( P 0.05). The positive rate in highly, moderately and poorly differentiated gastric adenocarcinoma was 40.00%(8/12), 55.56%(10/18) and 62.96%(17/27) respectively, which showed a trend of increase in poorly differentiated cancer, but there was no statistical significance among them ( P 0.05) . Conclusion Survivin is significantly increased in gastric cance but undetected in benign gastric ulcers. The expression rate of survivin is higher in advanced stages of gastric cancer and may indicate metastasis. Survivin may play an essential role in gastric carcinogenesis and serve as a marker for prognosis and therapy.
