Title: Effect of MTHFR Gene Polymorphism on Dynamic Monitoring of Plasma Homocysteine Level in H-type Hypertension by Combined Drug Intervention
Abstract: Objective To study the efficacy of enalapril-folic acid tablet in lowering plasma homocysteine(Hcy)and the early warning effect of methylenetetrahydrofolate reductase(MTHFR)gene polymorphism on monitoring its efficacy in patients with H-type hypertension. Methods Based on the community project of comprehensive prevention and treatment of stroke in Nanshan District, Shenzhen, we used polymerase chain reaction-restriction fragment length polymorphism method to detect MTHFR gene polymorphism in 150 patients with H-type hypertension and 80 controls. And the difference in Hcy with different genotypes was analyzed using circulating enzymatic method.After 4 weeks treatment with enalapril-folic acid tablets, the changes of plasma Hcy levels of 95 patients with H-type hypertension were determined. Results There were statistically significant differences in the plasma Hcy levels among CC and CT and TT genotypes in H-type hypertension; moreover, the Hcy level of TT genotype was the highest(P 0.01). After 4 weeks treatment with enalapril-folic acid tablets, the Hcy level of TT genotype in 95 patients with H-type hypertension was decreased significantly, and the difference was statistically significant(P 0.05). Conclusions TT genotype was homozygous mutant genotype on MTHFR gene of 677C/ T. TT genotype in H-type hypertension may be an important genetic factor for elevating plasma Hcy level. Taking enalapril-folic acid tablets can effectively reduce the high-risk occurrence of high Hcy in hypertensive patients, especially on TT genotype.
