Title: Choice of treatment for simple lower calyceal calculi
Abstract: 【Objective】To investigate the effective and secure treatment strategy of lower calyceal calculi.【Methods】106 patients with lower calyceal calculi(﹥or =10 mm) were divided into random two groups,58 pa-tients were treated were by extracorporeal shock wave lithotripsy(ESWL),48 patients were treated by mini percuta-neous nephrolithotomy(MPCNL).Analyzed the treatment result and calculate the stone-free rate.【Result】All cases had been followed-up for 1~3 months post-operation.There are 16 cases with remnant stone,14 cases with ineffi-ciency,28 cases stone-free after ESWL,the most remnant stone is 1.1 cm.The stone-free rate of the group of ESWL is 48.28%.In group of MPCNL,there are three cases with remnant stone after post-operation,the most rem-nant stone is 0.5cm.The stone-free rate of the group of MPCNL is 93.92%.There is statistically significant differ-ence of stone-free rate between two groups(P 0.05).Severe complication didn’t present and few patients with macroscopic hematuria in ESWL groups.Some complications like massive haemorrhage,renal parenchyma dilacera-tions,pleura damnification,etc,didn’t occur during operation.There are two cases with secondary haemorrhage and two cases with fever after operation of MPCNL,but they cured through expectant treatment.Three were no cases with blood transfusion and infection shock during or after the operation.【Conclusions】MPCNL can be used as the first choice of therapy for lower calyceal calculi(﹥or =10 mm),and MPCNL is a secure and effective therapy for patients with lower calyceal calculi,thus the treatment is satisfying.
