Title: Evaluation of hemodynamics and vascular imaging before and after thrombolytic therapy of acute cerebral infarction by 4D technic of 128-slice spiral CT
Abstract: Objective To explore the value of hemodynamics and vascular imaging before and after thrombolytic therapy of acute cerebral infarction by 4D technical of 128-slice spiral CT.Methods 15 patients with acute cerebral infarction treated between October 2011 and March 2012,were chosen who were in the onset of 24 hours,to receive 4DCT scan.After intravenous thrombolytic therapy,the patients reexamined by 4DCT scan within 72 hours to measure regional cerebral blood flow,regional cerebral blood volume,time to peak and mean transit time,then to get vascular imaging.The results were compared and analysed.Results Low-perfusion areas of brains and decreasing vessel branches were found on each patient.13 patients showed branch reduction in vascular infarction area,2 cases of cerebral imaging had not seen the obvious abnormity.After intravenous thrombolytic therapy,rCBF and rCBV of the marginal zones of low-perfusion areas,TTP and MTT increased,while vessel branches increased(rCBF before treatment(20.72±3.11) s,treatment after(38.33±8.07) s;rCBV before treatment(1.21±0.30) s,treatment after(1.98±0.38) s).7 patients reached peak time(TTP) shorten,8 patients TTP extended;6 patients MTT shorten,9 patients MTT extended;13 patients with vascular infarction area local branch increased;Statistical analysis showed that after fibrinolytic therapy all patients infarction fringe areais cerebral perfusion improved obviously,rCBF and rCBV and before thrombolysis were statistically significant difference(P0.05),the difference between before and after of TTP and MTT had no sense of statistics(P0.05).Conclusion Good response of hemodynamics and vascular imaging before and after thrombolytic therapy of acute cerebral infarction were shown by 4D technic of 128-slice spiral CT,which would provide accurate bases for evaluation of treatments.
