Title: Isoniazid in Original Treatment of Cavitary Tuberculosisl
Abstract: In this study of the original treatment of 223 patients with cavitary pulmonary tuberculosis, a 2drug regimen of ethambutol (EMB) plus isoniazid (INH) (regimen 1) was compared to a 3-drug regimen of streptomycin (SM) plus EMB plus INH (regimen 2) with 1 g of SM given daily for 90 days. The dose of EMB was 25 mg per kg for 90 days and 15 mg per kg thereafter, and the daily dose of INH was 450 mg. No patient had retrobulbar neuritis due to EMB. There were a few in stances of intolerance to INH that required its discontinuation. Drug reactions were most frequent with SM. In patients treated for 4 months or more, sputum cultures were still positive in 4 per cent of those given regimen I and 5 per cent of those given regimen 2. The 3-drug regimen had 2 advantages: sputum conversion was more rapid in the far advanced cases, and no patients were lost to treatment because of failure to improve or to demonstrate roentgenographic progression. There were no other significant differences in the therapeutic results from the standpoint of the bacteriologic and radiographic responses to the 2 regimens. However, significantly fewer drug related losses to the study were associated with the EMB-INH regimen.
