Title: Lipid and fatty-acid composition of plasma lipoproteins in cases of aortic atherosclerosis
Abstract: Cohn's method 10 has been used to obtain protein fractions IV + V and I + II + III from the plasma of normal young men and from men shown by aortography to be suffering from severe atherosclerosis of the aorta. Lipids extracted from these protein fractions (referred to for convenience as a- and β-lipids respectively) were separated into phospholipids, free cholesterol plus glycerides, free fatty acids and cholesterol esters; the fatty-acid composition of these four fractions was determined by gas-liquid chromatography and the composition of the phospholipids by chromatography on silica-impregnated paper. All the atherosclerotic subj ects were mildly to moderately hyper-β-lipoproteinaemic, the a-lipoprotein level (and composition) being the same as in normal controls. The β-lipids from subjects with aortic atherosclerosis were slightly modified in composition, having a higher sphingomyelin and lower free fatty acid content than those from the controls. The fatty-acid compositions of corresponding fractions (cholesterol esters, glycerides, etc.) of a- and β-lipids were virtually identical, both in normal and atherosclerotic individuals. In spite of a wide individual variation, a lower linoleic acid content of the cholesterol ester fatty acids of atherosclerotic subjects could be established. The possibility that the differences found were wholly or in part due to age differences between patients and controls cannot be excluded. Not all the lipid differences observed parallel those found by other authors using clinical symptoms of ischaemic heart disease as criteria for the existence of atherosclerosis. Les lipides extraits de ces fractions proteiques (appelés pour simplifier, respectivement lipides-a et -β) sont séparés en phospholipides, cholesterol libre plus glycerides, acides gras libres et cholesterol estérifié; la composition en acides gras de ces 4 fractions est déterminée par chromatographie gazeuse et la composition des phospholipides par chromatographie sur papier imprégné d'acide silicique. Chez tous les sujets atherosclereux les μ-lipoprotéines du sang sont legerement ou modérément augmentées; le taux ainsi que la composition des a-lipoproteines est le même que chez les temoins normaux. La composition des lipides-β provenant des sujets qui ont une atherosclerose aortique est legerement modifiée et revele un taux plus élevé de sphingomyélines et plus bas d'acides gras libres que chez les temoins. La composition en acides gras des diverses fractions (esters du cholesterol, glycerides, etc.) provenant des lipides-a et -β) est virtuellement identique, chez les sujets normaux et atherosclereux. En dépit de grandes variations individuelles on a pu observer une diminution du taux d'acide linoléique contenu dans les esters du cholesterol des sujets atherosclereux. Il est possible que les differences trouvees soient expliquées en partie ou en totalité par les differences d'âge des patients et des temoins. Les differences observées ne sont pas toutes conformes a celles trouvees par les auteurs qui se fondent sur les signes cliniques comme critères de l'existence d'une atherosclerose. Unter Benutzung der Cohn'schen Methode 10 wurden aus dem Plasma von gesunden jungen Männern and von männlichen Patienten mit einer aortographisch nachgewiesenen schweren Atherosklerose der Aorta die Proteinfraktionen IV + V and I + II + III gewonnen. Die aus diesen Proteinfraktionen extrahierten Lipide (hier als a- and β-Lipide bezeichnet) wurden in Phospholipide, freies Cholesterin and Glyceride, freie Fettsäuren and Cholesterinester getrennt, die Fettsäurenzusammensetzung dieser 4 Fraktionen durch Gaschromatographie and die Zusammensetzung der Phospholipide durch Chromatographie an silicagelimprägniertem Papier ermittelt. Bei allen atherosklerotischen Personen bestand eine geringe bis mässige Hyper-β-Lipoproteinämie, während Höhe (und Zusammensetzung) der a-Lipoproteine sich wie bei den normalen Kontrollen verhielten. Die β-Lipide der Patienten mit Aortenatherosklerose waren in ihrer Zusammensetzung geringfügig verändert, sie hatten einen höheren Sphingomyelingehalt and einen niedrigeren Gehalt an freien Fettsäuren als bei den Kontrollen. Die Fettsäurenzusammensetzung entsprechend der Fraktionen der a- and β-Lipide (Cholesterinester, Glyceride, usw.) war bei den Gesunden and bei den Patienten im wesentlichen identisch. Trotz erheblicher individueller Variationen konnte in den Cholesterinestern der Atherosklerotiker ein niedrigerer Linolsäuregehalt festgestellt werden. Die Möglichkeit, dass die nachgewiesenen Unterschiede ganz oder teilweise durch die Altersunterschiede zwischen Patienten and Kontrollen bedingt sind, konnte nicht ausgeschlossen werden. Nicht alle beobachteten Lipiddifferenzen entsprachen den Feststellungen anderer Autoren, die als Kriterium fur das Bestehen einer Atherosklerose die klinischen Symptome einer ischämischen Herzerkrankung benutzten.
