Title: Abnormalities of monocyte chemotaxis following thermal injury
Abstract: Sepsis is the leading cause of morbidity and mortality in severely burned patients. Since normal monocyte function is essential to host defense, the chemotactic responses of monocytes from 25 burned patients were evaluated in vitro. Patients with burns involving <20% of the total body surface had normal monocyte chemotaxis, while monocytes from patients with burns ≥20% had depressed chemotaxis. This defect was most pronounced in patients with burns ≥40%. Analysis of the data as a function of time revealed that monocyte chemotaxis from patients with ≥40% burns was initially near normal, reached a nadir at approximately Day 45, and gradually returned to normal. These studies document that depressed monocyte chemotaxis occurs as a consequence of thermal trauma and indicate that the magnitude of the abnormality is proportional to burn size.
