Title: A Case of Intraductal Papillary Mucinous Carcinoma of the Pancreas
Abstract: A 73-year-old man admitted for abdominal pain was diagnosed as having a main-duct intraductal papillary mucinous tumor (IPMT). Although pylorus-preserving pancreatoduodenectomy with pancreaticogastrostomy reconstruction was performed, panceatitis eventually appeared in the intra pancreatic duct. Finally, on the second occasion, total resection of the remnant pancreas was performed. The histopathological finding following the first surgery was intraductal papillary mucinous adenoma (IPMA) of the pancreas. The remnant pancreas at the second operation contained intraductal papillary mucinous carcinoma (IPMC). This case suggests that main-duct IPMT with dilatation of the whole main pancreatic duct has great potential for IPMC at the time of diagnosis and may require total pancreatectomy.
