Abstract: INTRODUÇÃO:O diabetes materno pode determinar hipertrofia miocárdica (HM) e cardiopatias estruturais (CE) no feto.O diagnóstico precoce destas alterações interfere na morbi-mortalidade destes pacientes.Objetivo: Determinar a prevalência e associação da HM e de CE em fetos de mães com diabetes tipo I, II ou gestacional.MÉTODOS: entre janeiro de 1990 e dezembro de 2003 foram avaliados 2897 fetos de mães com diabetes tipo I, II, ou gestacional, submetidas a ecocardiograma prénatal.O diagnóstico de HM septal foi realizado quando a espessura do septo interventricular fosse maior que dois desvios-padrão para a idade gestacional.Foram excluídos 303 pacientes, devido a dados imcompletos nos prontuários.A análise estatística utilizou o teste do qui-quadrado, com um alfa crítico de 0,05.RESULTADOS: A idade materna média foi 32,19 ± 6,38 anos e a idade gestacional 31,59 ± 4,18 semanas.A prevalência global de HM foi de 28,8% (833 fetos).Analisando-se o tipo de diabetes e a presença de hipertrofia, esta estava presente em 193 fetos com diabetes prévio (44,06%) e em 640 com diabetes gestacional (29,7%) (p<0,001).Dos 833 fetos com HM, 24 (2,88%) tinham associação com CE, sendo a comunicação interventricular a mais prevalente.Dos 1761 fetos sem hipertrofia septal, 46 (2,61%) tinham CE, sendo também a comunicação interventricular a mais prevalente.Esta diferença não foi estatisticamente significativa (p=0,39).Em relação ao tipo de diabetes (I, II ou gestacional), não foram encontradas diferenças significativas quanto à presença de CE (p=0,059).
