Abstract: Laparoscopic anterior Heller myotomy has been widely used to treat achalasia. For patients with failed anterior myotomy, secondary treatment with posterior esophageal peroral endoscopic myotomy (POEM) provides an efficacious therapy option; however, there is a paucity of literature on these patients. In patients with prior interventions, submucosal fibrosis may prevent tunneling and lead to premature POEM termination. While early research showed longer POEM procedure times for patients with prior treatments, more recent data showed no difference.
