Title: First experience of the study of prevalence of the Helicobacter pylori-negative peptic ulcer in Lviv Region
Abstract: The prevalence of Helicobacter pylori-negative peptic ulcers of the stomach and duodenum in Ukraine has not been studied yet. The lack of accounting for such patients, lack of knowledge of risk factors for the development of this variant of the disease, insufficient experience in the disease treatment and prevention causes serious problems, especially at the stage of initial request for medical care. The known Clinical Protocol on all three levels of medical care for this category of patients (2013) is imperfect and requires revision with significant additions. Based on the survey of 264 patients, seeking for medical care for the gastroduodenal peptic ulcers during 4 months of 2018, the ratio of Helicobacter pylori-positive and negative patient statuses was established and an attempt was made to establish possible risk factors for a non-infectious variant of the disease. Thus, the frequency of gastroduodenal zones ulcers in patients with Helicobacter pylori-negative status was revealed as 23.1 %, Helicobacter pylori-positive — 39 %, and erosion — respectively 76.9 % and 60.9 %. In case of Helicobacter pylori-positive status, the duodenal localization dominated (29.7 % against 10 %), and at Helicobacter pylori-negative status the gastric localization prevailed (67.7 % against 47.6 %). Among risk factors, contributing the development of Helicobacter pylori-positively ulcers, smoking and alcohol abuse (67.2 % and 57.4 %) were the most prevalent. The main risk factor of Helicobacter pylori-negative option was the increased body mass index (average 25.9). Liver cirrhosis as a risk factor was defined in 7.7 % of patients with a Helicobacter pylori-negative. The gastroduodenal erosive-ulcer lesions, combined with chronic pancreatitis, was established in 7.8 % of patients with the Helicobacter pylori-positive status.
