Title: Von Willebrand's Disease: Assessment, Analysis, and Associated Dental Management Guidelines
Abstract: The von Willebrand factor is made of several identical subunits and mutations in the von Willebrand factor (vWF) gene, which results in the different types of von Willebrand disease (vWD). This disease is categorized into type 1,2, and 3. Type 1 vWD is the most common and mildest form of the disease; 85% of patients diagnosed with vWD have type 1 vWD. Type 3 is the most severe form of von Willebrand disease. It is associated with a complete absence of production of vWF. Excessive bruising at unusual sites, mucous membrane bleeding, heavy nosebleeds, heavy menstrual cycles, and prolonged oozing following extraction(s) are some of the common symptoms experienced with vWD. All these symptoms relate to problems with primary hemostasis. This chapter provides dental guidelines for vWD. Primary therapy consists of giving desmopressin (DDAVP), IV, intranasal, or PO (oral). Secondary therapy is provided with factor concentrate or cryoprecipitate. Cryoprecipitate contains Factor VIII, vWF, and fibrinogen.
