Title: Anamnestisches Screening bei symptomatischen Patientinnen mit und ohne Endometriose
Abstract: Fragestellung: Die sichere Diagnose der Endometriose ist bis jetzt nur invasiv mittels Laparoskopie, idealerweise gesichert durch histomorphologische Untersuchungen des Gewebes, möglich. Exakte Anamneseerhebung kann jedoch im Vorfeld wertvolle Informationen liefern; mehrere Studien haben bisher Hinweise für eine mögliche Unterscheidung zwischen Endometriosepatientinnen und symptomatischen Patientinnen ohne Endometriose auf Basis von anamnestischen Parametern gezeigt. Methodik: In der vorliegenden prospektiven Studie wurde 158 symptomatischen Patientinnen, die wegen Dysmenorrhö, Dyspareunie, chronischem Unterbauchschmerz und/oder Infertilität in die Klinik zur Laparoskopie kamen, ein Anamnesefragebogen und ein psychologischer Fragebogen (Stressverarbeitungsfragebogen, SVF) ausgehändigt. Durch Laparoskopie und gegebenenfalls histologische Diagnosesicherung erfolgte die Zuordnung in die Gruppe der Endometriosepatientinnen oder in die Kontrollgruppe. Ergebnisse: Die Prävalenz für Endometriose in der Studienpopulation war 57,6 % (91/158). Endometriosepatientinnen hatten signifikant weniger Schwangerschaften (p < 0,05), signifikant weniger Geburten (p < 0,01) und die Zyklusdauer war signifikant kürzer (p < 0,01). Signifikant mehr Endometriosepatientinnen gaben den 1. Tag der Menstruation als intensivsten Schmerztag an (p < 0,001) und Endometriosepatientinnen nahmen signifikant häufiger Schmerzmittel gegen ihre Beschwerden ein (p < 0,05). In der Endometriosegruppe beschrieben mehr Patientinnen Schmerzen bei der Defäkation (p = 0,07) und vermerkten häufiger einen Zusammenhang mit der Menstruation (p = 0,06). Die Tendenz zur Einnahme von Suchtstoffen in Stress- bzw. Konfliktsituationen als Parameter des SVF war mit p < 0,05 signifikant höher als bei den Patientinnen der Kontrollgruppe. Schlussfolgerung: Anamnestische Untersuchungen und die ergänzende Anwendung von psychologischen Fragebogen liefern wertvolle Informationen im Vorfeld der diagnostischen/kurativen Operation und können hinweisend sein für das Vorliegen einer Endometrioseerkrankung und unterstützen somit die optimierte Beratung von symptomatischen Patientinnen in der Phase der Entscheidungsfindung zu einer operativen Abklärung.
