Title: Efficacy and Safety of Saxagliptin in Type 2 Diabetes Patients Poorly Controlled with Insulin
Abstract: Objective The course of the disease was observed in patients with type 2 diabetes use insulin more than 5years of non- union dipeptidyl peptidase 4( DPP- 4) inhibitors,saxagliptin when added with poor glycemic control,blood sugar control and improve islet function effect. Methods A total of 64 patients with type 2 diabetes who were admitted to our hospital from January 2012 January 2013,then the patients were randomly divided into the original program( control group) and processing with saxagliptin group( experimental group). The control group continued to increase according to blood glucose blood glucose control with insulin to achieve the stated objectives,the experimental group in the original scheme based on the use of saxagliptin,saxagliptin dose adjustment based on insulin and blood glucose. Comparison of the two groups before and after treatment 12 weeks of fasting blood glucose,postprandial 2h plasma glucose,glycated hemoglobin( HbA1c),fasting C- peptide,fasting insulin,homeostasis model assessment of insulin resistance index( HOMA- IR),insulin daily dose,body weight and changes in hypoglycemic events. Results Two groups of patients after 12 weeks of treatment,fasting blood glucose,postprandial 2h plasma glucose,HbA1c,HOMA- IR than before treatment decreased,the difference was statistically significant( P 0. 05); The experimental group changes in body weight before and after treatment,the difference was not statistically significance( P 0. 05). After 12 weeks of treatment,The experimental group fasting blood glucose test,2h postprandial blood glucose,HbA1c,HOMA- IR,fasting C- peptide and insulin use were lower than the control group,the difference was statistically significant( P 0. 05). Conclusion Non- insulin combined DPP- 4 inhibitor therapy in poor glycemic control in patients with type 2 diabetes plus saxagliptin,significantly reduced the amount of daily insulin use,without increasing the risk of hypoglycemia premise effective control of blood glucose to improve insulin resistance.
