Title: A Study on Function of Hypothalamus-pituitary adrenal Axis in Type 2 Diabetic Patients with Depression
Abstract: Objective:To investigate the variations of hypothalamus pituitary adrenal axis(HPA) in type 2 diabetic patients and those with depression and to evaluate their clinic significance.Methods:Blood samples of 64 cases of type 2 diabetes mellitus(DM),including 32 patients without depression(DM group),32 patients with depression(DD group) and 32 cases of healthy subjects (control group) were tested for basal serum cortisol levels(F,08∶00am,16∶00pm),serum cortisol after low dose of dexamethasone suppression test(DST),fasting plasma glucose (FPG),fasting serum insulin(FINS),Hemoglobin A 1c(HbA 1c),24h urinary free cortisol(UFC).Results:①Compared with the control group,the levels of F(08∶00am,16∶00pm),FPG,FINS,HbA 1c,UFC,insulin resistance(IR,calculate by the HOMA model )in the 64 cases of patients with type 2 DM were significantly higher( P 0 01). 33 cases with non supressive response were found in the 64 cases type 2 DM,but there was no case in the control group.②The levels of F,UFC,FPG,FINS,HbA 1c,HOMA IR,the number of cases with non supressive response were higher in DM group than that in the DD group ( P0 05,P 0 01).③24h UFC in type 2 DM was positive correlated with HbA 1c( P 0 01),HOMA IR( P 0 05).Conclusion:The function disturbance of HPA in patients of type 2 DM exacerbats disorders of glucose metabolism and insulin resistance,results in deterioration of disease.Depression play an important role in the development and prognosis of DM.
