Abstract:Objectives:To study the alteration of respirating function produced by artificial pneumoperitoneum and Trendelenburg position during the procedure of gynecological laparoscopic operation.Methods:40 ca...Objectives:To study the alteration of respirating function produced by artificial pneumoperitoneum and Trendelenburg position during the procedure of gynecological laparoscopic operation.Methods:40 cases underwent gynecological laparoscopic operation were divided randomly into two groups:20 cases of the peridural anesthesia and the rest of general anesthesia.ASA I grade.On operation the intra abdominal pressure was kept 1.6 kPa and the patientsposition was changed from horizontal position to Trendelenburg position(head down 20°).Results:In the peridural anesthesia group,V Tand MV were increased after artificial pneumoperitoneum and Trendelenburg position.P ET CO 2 values in the peridural anesthesia group were higher than that in the general anesthesia group and SpO 2 values were lower than that in the general anesthesia group.In general anesthesia group,after artificial pneumoperitoneum P PEAK was increased and C was decreased.In Trendeleburg position P PEAK was increased further.Every period after pneumoperitoneum and changing position all pH values( P 7.350)in the peridural anesthesia group were lower than that in the general anesthesia group.In the peridural anesthesia group PaCO 2values were higher than that in the general anesthesia group.Conclusions Artificial pneumoperitoneum and Trendelenburg position in gynecological laparoscopic operation may cause the decrease of C,leading to the clear decrease of lung effective ventilatory volume fall.Under the peridural anesthesia,the patients had a tendency of respiratory acidosis and hypoxemia.Under the general anesthesia the patients could avoid CO 2stored.It has the higher safety.Read More
