Title: Short-term outcome in elderly patients undergoing primary PCI for ST-segment elevation myocardial infarction
Abstract: Objective To determine the 30 day outcome and its influencing factors in elderly patients with ST-segment elevation myocardial infarction(STEMI)undergoing primary PCI.Methods Of the 870 STEMI patients undergoing primary PCI,the clinical and angiographic features,medical treatment,procedural characteristics and 30 day outcome were compared between the elderly group(≥80years old,n=92)and the control group(45-79 years old,n=778).The predictors of 30 day all-cause death were investigated.Results Compared with the control group,the elderly group had longer hospital stay(13dvs 10 d,P=0.000),lower LVEF(55%vs 59%,P=0.007),higher incidence of cardiogenic shock(18.5%vs 6.0%,P=0.000),acute kidney injury(22.8%vs 8.6%,P=0.000)and 30 day all-cause death(19.6% vs 5.0%,P=0.000).Multivariate analysis revealed that age≥ 80 years was one of the independent predictors of 30 day allcause death in all STEMI patients(HR=1.898,95%CI:1.030-3.497,P=0.040);while female gender(HR=4.894,95%CI:1.572-15.234,P=0.006)and acute kidney injury(HR=10.155,95%CI:3.140-32.837,P=0.000)were independent predictors of 30 day all-cause death in the elderly group.Conclusion Even with primary PCI as their reperfusion strategy,elderly STEMI patients had higher 30 day all-cause death compared with the control group.
