Title: Individual repair technique for unilateral microform cleft lip
Abstract: Objective To explore individual design for the unilateral microform cleft lip and repairing method of philtrum ridge.Methods For preserving more intact skin on the cleft side,the individual designs were performed to repair the microform cleft lip.The local Rose incision was applied for the patients whose the differences of lip heights between the non-cleft side and the cleft side were less than 2 mm;If the difference was longer than 2 mm,a isosceles triangular skin flap which the length ratio was 2/3 of its base and side and a local reverse V incision for eliminating car-ear were performed for the patients with the differences of lip heights between the non-cleft side and the cleft side were more than 2 mm.A crossed triangular flap on the vermilion of the lip was also designed in order to remove the notch on the red lip.Orbicularis oris muscle was separated from the skin and mucosa through the incisions to basis nasi,beginning inferiorly and extending as cephalad as possible.Cuimp orbicularis oris muscle was cut on the sagittal plane;the muscles were separated on the frontal plane and then the philtrum part of obicularis oris muscle was crossed and overlapped to augment thickness of muscles in philtrum ridge;the lateral cutaneous triangular flap and vermilion of lip flap were also inserted to rebuild the curve of labial arch and fullness of vermilion of lip.Rhinoplasty could be carried out simultaneously according to the degree of deformity and the request of patients.Results Individual designs for repairing the microform cleft lip could preserve 1/3 to 2/3 intact skin and rebuilt symmetry of labial arch,form of philtrum ridge and fullness of vermilion of lip.Conclusion All patients with unilateral microform cleft lip were satisfied with the individual technique which can recover efficiently symmetry and fullness of the upper lip and reduce scar length.
