Title: Effect of endoscopic therapy and omeprazole injection on patients with active bleeding peptic ulcer
Abstract: Objective To determine the effect of endoscopic injection and omeprazole on control of bleeding in peptic ulcer patients with active bleeding or nonbleeding visible vessels. Mehtods One hundred and one peptic ulcer patients with active bleeding or nonbleeding visible vessels (ForrestⅠ a,Ⅰ b,Ⅱ a) were randomly enrolled into three groups. In the first group, the patients were given endoscopic injection of hemostatic, and then followed by a 40mg omeprazole intravenous injection twice daily for 5 days. In the second group, the patients were given endoscopic injection hemostasis therapy, and then followed by a 40 mg omeprazole intravenous injection once daily for 5 days. While in the third group, the patients were only given 40 mg omeprazole intravenous injection twice daily for 5 days. Results Rebleeding occurred in 3 patients (8.6% ), 9 patients (28.1% ) and 15/34(44.1% ) in the first, second and third group respectively, P 0.05. There was a tendency for patients in the first group to acquire less time for hemostasis and hospital stay than those of the other two groups. The blood transfussion, surgery rate and mortality rate were similar between these three groups. Conclusion Endoscopic therapy combined with omeprazole (80 mg/d) can effectively reduce the rebleeding rate, hospital stay and hemostasis time in patients with bleeding peptic ulcer. It was recommended for the treatment of patients with bleeding peptic ulcer.
