Title: Sinonasal type hemangiopericytoma: a clinlicopathologic study of five cases
Abstract: Objective To study the clinicopathological features,immunophenotype and differential diagnosis of sinonasal type hemangiopericytoma(SNTHPC).Methods The clinicopathological and immunohistochemical features were reevaluated in 5 cases of SNTHPC,and 6 cases of solitary fibrous tumour(SFT) in soft tissue were used as control.Results Amongst the 5 patients studied,4 were female and 1 was male.The median age of patients was 54 years old.The left nasal cavity was involved in 2 cases and the right in 3.The commonest clinical presentation was nasal obstruction and/or opistaxis.The median size of tumor was 2.6 cm.Histologically,the tumors were submucosal and unencapsulated,and showed a mixture of short fascicular,storiform and whorled growth patterns,and alternating hypercellular and hypocelluar arrangements in 2 cases.The tumors were rich of thin-walled,variable-sized vascular channels and focal hyalinization changes in 2 cases.The tumor cells had round-shaped to oval or spindle with clear-appearing cytoplasm and distinct cell borders.Nuclear pleomorphism was mild and mitotic figures were identified 1 to 3 per 10 high power fields(HPF),atypical mitoses were not present.Extravasated erythrocytes were also identified in all cases,and haemorrhage in 3 cases.The presence of an infilammatory infiltrate composed of mast cells,eosinophils and lymphocytes was nearly ubiquitous present in all tumors,but necrosis was absent.Immunohistochemically,all neoplasms positive for vimentin and SMA,D2-40 were detected in vascular channels of all SNTHPC patients examined.In 6 cases of solitary fibrous tumor,vimentin,CD34,CD99 and bcl-2 were all diffusely positive and SMA reactivity was focally present in 2 cases.Follow-up information was available in all cases,ranged from 6 months to 2.5 years,revealed only local recurrences in 1 case.ConclusionSNTHPC is a rare neoplasm in nasal cavity or paranasal sinus in adults,and shows a potential for local recurrence but infrequently metastatic disease.Its correct diagnosis and differential diagnosis depend on the combination of routine histopathological analyses and immunohistochemistry.
