Title: Study on diagnosis and treatment of intracranial infections in patients with severe brain injury after decompressive craniectomy
Abstract: OBJECTIVE To explore the clinical manifestations of the severe brain injury patients with intracranial infections after decompressive craniectomy and observe the risk factors so as to improve the level of clinical diagnosis and treatment of the disease.METHODS Totally 195cases of severe brain injury,who underwent the decompressive craniectomy from Dec 2010to Feb 2013,were retrospectively analyzed,then the clinical manifestations,laboratory examination,risk factors,and therapeutic effect of 35subjects complicated with intracranial infections were analyzed and summarized.RESULTS Of totally 195patients undergoing the decompressive craniectomy,the intracranial infections occurred in 35cases with the infection rate of 17.9%.The high fever and neck stiffness were the major clinical manifestations,the meningeal irritation was the positive sign,and the imaging was mainly characterized by enlarged ventricles uniformity;the turbid cerebrospinal fluid or purulent change and the elevated level of CSF white blood cells or glycoprotein content were the main characteristics.The Staphylococcus aureus was the main species of pathogen causing infection.The risk factors included the long operation duration,multiple complications,low GCS,leakage of cerebral spinal fluid,ventricular drainage or intubation,open injury,and long-term use of antibiotics(P0.05).The effective rate of the symptomatic treatment was 82.9%,and the mortality was 11.4%.CONCLUSIONThe severe brain injury patients with intracranial infections have obvious clinical manifestations after the decompressive craniectomy,the risk factors are complex;it is necessary for the hospital to take comprehensive treatment measures.
