Title: Clinical effect of laparoscopic total gastrectomy for obese patients with gastric cancer
Abstract: Objective: To explore the clinical influence of obese for patients with gastric cancer who underwent laparoscopic D2 curative resection and total gastrectomy. Methods: Clinical data of 164 obese patients( BMI≥25 kg / m2) who underwent D2 curative resection and total gastrectomy from Jan. 2004 to Sep. 2014 were retrospectively analyzed,including 49 patients in laparotomy group and115 patients in laparoscopic group. Clinical data,intraoperative and postoperative situation,the number of lymph node dissection,and complications of 2 groups were compared. Results: Compared with laparotomy group,the laparoscopic group's operation time was longer[( 270. 2 ± 73. 5) min vs.( 240. 4 ± 53. 2) min],the amount of intraoperative bleeding was less [( 90. 0 ± 38. 2) ml vs.( 206. 4 ±50. 5) ml],anal exhaust after operation was earlier [( 2. 8 ± 1. 3) d vs.( 3. 4 ± 1. 5) d],consumption of liquid food was earlier [( 3. 5± 1. 4) d vs.( 4. 3 ± 1. 7) d],ambulation was earlier [( 3. 0 ± 1. 8) d vs.( 4. 7 ± 2. 7) d],the differences were statistically significant( P 0. 05); the average number of lymph node retrieved [( 21. 2 ± 4. 2) vs.( 23. 2 ± 9. 7) ],complication rate( 17. 4% vs. 20. 4%),and the hospital stay after operation [( 10. 5 ± 9. 5) d vs.( 12. 3 ± 3. 7) d]were not statistically significant different( P 0. 05). Conclusions: Laparoscopic total gastrectomy for gastric cancer in obese patients has longer operation time,while its postoperative recovery is faster,and doesn't increase the incidence of postoperative complications,is a safe and effective therapeutic method.
