Title: Application of autogenous cartilage combined with ePTFE in rhinoplasty
Abstract: Objective To introduce a new method in rhinoplasty in order to shape the ideal nose with the combined application of septal cartilage of nose,ear cartilage,and expanded polytetrafluoroethylene(ePTFE)graft.Methods With combined incisions of midcolumellar gullwing and those inside the nostrils,the columellar flap is elevated with blunt scissor dissection to the superior aspect of the medial crura.The nasal tip skin is then elevated off the alar cartilages with gentle retraction and scissor dissection.The nasal dorsum is further exposed to harvest the septal cartilage. Sculped septal cartilage is inserted between the nasal alar cartilages,which is the new nasal columella.The harvested ear cartilage is sculped as shieldlike,and sutured in front of the nasal columella.The shaped ePTFE is inserted into the nasal dorsum and fixed before the closure of the incision.Results Satisfactory results are obtained in all 20 cases with natural and beautiful looking.No complications happen only with a longer recovery time.Conclusins This method with combined application of septal cartilage of nose,ear cartilage,and expanded polytetrafluoroethylene(ePTFE)graft in rhinoplasty is one of the good options to get the ideal shape of nose.
