Title: Clinical experience in digestive reconstruction of P-type Roux-en-Y esophagojejunostomy following total gastrectomy in patients with gastric cancer
Abstract: Objective To investigate an ideal way to reconstruct the digestive canal after total gastrectomy.Methods The clinical data of 238 cases of gastric cancer confirmed by pathology were analyzed retrospectively between Jan. 2000 and Dec. 2007,and every patient was subjected to P style of total gastrectomy and digestive canal reconstruction by using jejunum.The complications,effect of the bag for the stomach,and the body weight were observed.Results Of the 238 cases,there was no death causalgia behind sternum.Two cases had anastomotic leakage,2 cases had subphrenic abscess and 2 cases had fat liquefaction.After barium meal examination,the incidence of barium staying in P jejunum more than 1h and 2 h was 60% and 40% respectively.During the follow-up period in the patients with the survival time more than one year,the meal habit was recovered.In 40% of 238 cases the body weight was kept stable,and in 30% of them the body weight was increased after operation.Conclusion P style operation in gastric cancer is easy and safe to perform.Reflux esophagitis and dumping syndrome seldom developed.P jejunal ansa basically plays the role in preservation and nutrition of stomach,so this is an ideal way to reconstruct the digestive canal after total gastrectomy.
