Title: Comparison of valve replacement and valve plasty in patients with moderate to severe functional tricuspid insufficiency
Abstract: Objectives To compare the clinical effects of tricuspid valve replacement and tricuspid valve plasty in patients with moderate to severe functional tricuspid insufficiency. Methods Of the 1 679 patients with moderate to severe functional tricuspid valve insufficiency, according to their different operational methods, the patients were randomly selected and divided into tricuspid valve replacement group(47 cases) and tricuspid valve plasty group(47cases). Duration of aortic cross-clamping, duration of cardopulmonary bypass(CPB), intensive care unit(ICU) length of stay, 3-year recurrence rate, 1-year and 3-year survival rates of the two groups were observed and compared.Results(1) Average age of tricuspid valve replacement group was significantly lower than that of tricuspid valve plasty group [(41.24 ±9.34) years vs.(46.54 ±11.55) years, t =2.4 462, P =0.0 163 ]; tricuspid regurgitant area in tricuspid valve replacement group was significantly larger than that in tricuspid valve plasty group [(31.99±14.43) cm2 vs.(29.41 ±14.94) cm2, t =2.1 814, P =0.0 317 ].(2) Duration of aortic cross-clamping, duration of CPB and ICU length of stay of tricuspid valve replacement group were significantly longer than those of tricuspid valve plasty group[(86.34±20.34) min vs.(81.24±33.23) min, t=2.6 691, P=0.0 012;(153.45±52.34) min vs.(121.24±54.62) min,t =2.9 190, P =0.0 044;(101.67 ±44.32) h vs.(81.25 ±43.32) h, t =2.258 9, P =0.026 3 ].(3) Recurrent rate of tricuspid valve replacement group was significantly lower than that of tricuspid valve plasty group [19.15%(9 / 47) vs.63.83%(30 / 47), t =19.325 9, P=0.000 0 ]. Conclusions The early clinical effect of tricuspid valve plasty is better than that of tricuspid replacement, but the 3-year recurrence rate of tricuspid replacement is better than that of tricuspid valve plasty.
