Title: Effect assessment of the clinical pathway management controlling the blood pressure of the patients with hypertension in the community
Abstract: Objective: To evaluate the effect that Tang Qiao community health service center took the clinical pathway management mode to control the blood pressure of the hypertensive patients during 2010-2011. Method: The data of blood pressure and heart rate of 658 cases with hypertension in 2011and the first half of 2012 were compared, stratified and analyzed with gender and age. Result: The standard rates of blood pressure of systolic blood pressure were 90.00% and 89.00% and diastolic blood pressure 99.00% and 98.00%. The standard rates of heart rate were 100.00% and 96.00%. There was no difference in the average blood pressures and heart rates of two years (P0.05). Conclusion: After implementation of the clinical pathway management mode, the patients’ compliance and the level of standardized management of hospital have been improved.
