Title: A study on the clinical significance of abnormal myocardial blood supply in homozygote patients with familial hypercholesterolemia by myocardial perfusion imaging
Abstract: Objective: Investigating the effect and clinical value to myocardial blood supply in homozygous patients with the progress of familial hypercholesterolemia( FH) by myocardial perfusion imaging( MPI).Methods: As the objects,28 FH patients diagnosed by gene underwent drug induced stress / rest MPI. Put the patients into 2 groups according to whether the myocardial blood supply is normal or not and analyze the basic data,blood lipid and MPI data of patients statistically. 8 patients were followed in 1-2 years. Results: Among 28 patients,17 males and 11 females,mean age( 12. 12 ± 6. 4) years,TC( 16. 00 ± 2. 92) mmol / L,LDL-C( 13. 82 ± 2. 73) mmol/L; MPI showed 15 negtive( male/female = 11 /4) and 13 positive( male/female = 6 /7).ST segments of ECG moved down in 6 patients in stress test. There were myocardial ischemia in 29 myocardial segments( 13. 12%) in all 221 ones of 13 positive patients with MPI,and among 29 myocardial segments,21( 21 /29,72. 41%),5( 5 /29,17. 24%),3( 3 /29,10. 34%) ones belonged to the blood-supply areas of the left anterior descending,the left circumflex artery and the right coronary artery respectively. In the positive and negtive groups of MPI,there were no significant differences in the patients of age,height,weight,BMI,treatment time,gender,moving down of ST segment in ECG,TC,LDLC,TG and HDLC. The MPI of 8 followed patients showed that: the results of 4 patients who underwent MPI with no myocardial ischemia previously were still negtive,and with the positive MPI patients,the area,extent of ischemia in 3 patients had no change,1 patient got more serious myocardial ischemia and was dead subsequently because of heart failure. Conclusion: The ischemia area with FH homozygous patients was easy uccur in left anterior descending artery,and the myocardial ischemia had no obvious correlation with age,height,weight,treatment time and blood lipid and so on. They should be undergone MPI as earlier as possible to evaluate cardiovascular disease risk stratification,treatment effectiveness and prognosis.
