Abstract: Objective To provide the basis of imaging anatomy of the sphenoid sinus for endoscopic sphenoid sinus surgery and endoscopic transsphenoidal surgery.Methods The sphenoid sinuses of 100(200 sides)subjects with normal nose and sinus were examined and observed by dual source computed tomography(DSCT) through three-dimensional reconstruction method.Results The distance from the root of columella nasi to the medial edge of the lower pole aperture of sphenoidal sinus was(58.10 to 92.80) mm,(69.97±5.33) mm on average ;the sagittal angle of the connection between the nasal floor and the sagittal plane was(18.90 to 47.70)°,(31.17±6.50)° on average;the incline angle of the connection between the two sides and the median sagittal plane was(1.50 to 9.20)°,(4.13±1.47)° on average;The distance from the root of columella nasi to the intersection point of posterior pharyngeal wall tangent and the inferior sphenoid sinus was(76.50 to 95.60) mm,(83.89±3.98) mm on average;the sagittal angle of the connection between the nasal floor and the sagittal plane was(6.20 to 31.00)°,(13.20±5.77)° on average;The distance from the root of columella nasi to the intersection point of maximum transverse diameter and the sphenoid sinus lateral wall of sphenoid sinus was(62.40 to 89.60) mm,(76.40±5.24) mm on average,the sagittal angle of the connection between the nasal floor and the sagittal plane was(8.40 to 40.40)°,(31.74±5.41)° on average,the incline angle of the connection between the two sides and the median sagittal plane was(4.40 to 21.00)°,(12.62±2.99)° on average;The distance between the median sagittal plane of the craniocerebral and the intersection point of maximum transverse diameter and the lateral wall of sphenoid sinus was(6.10 to 22.90) mm,(16.39±3.69) mm on average;The relationships of sphenoid sinus,ethmoid and optic canal were adjacent-types(77 sides,accounting for 38.50%);inrush-types(57 sides,accounting for 28.50%) and exposed-types(66 sides,accounting for 33.00%).Conclusions Three-dimensional reconstruction of DSCT could accurately and effectively measure the sphenoid sinus anatomy in the operative approach in the key structure of the endoscopic sphenoid sinus surgery and endoscopic transsphenoidal surgery,which was of great value in the space location and the prevention of complications.
