Title: Anatomic features of the confluence of the common iliac veins and its clinical significance
Abstract: Objective:To provide anatomic data of vessels at the anterior aspect of lumber vertebrae for transperitoneal approach lumbar anterior minimally invasive surgery. Methods:This study investigated 30 human cadavers and 120 patients' magnetic resonance images,to study the major vascular anatomy of the anterior aspect of lumber vertebrae,and observe the relationship of the confluence of the common iliac veins (CCIV ) with vertebral body or disc,as well the relationship of left iliac vein and L5~S1 disc. Results:CCIV ranged from L4 to S1,mostly at the level of L5 and L5~S1(87.5%). During the procedure of the anterior lumbar interbody fusion at the level of L5~S1,the site of vascular working window depended on the relationship of left iliac vein with L5~S1 intervertebral disc. Mostly,left common iliac vein(88.3% ) located at 30°~90° site of L5~S1 disc. Conclusions:The variability of the common iliac veins is common. positions of CCIV and left iliac vein increase difficulty of the anterior L5-S1 approach. For elevating the safety of operation,it is important to know the relationshiop of vascular structures with vertebral body under MRI before operation.
