Title: Comparison of clinical efficacy between laparoscopic and open D_2 radical gastrectomy for the aged distal gastric cancer
Abstract: Objective: To compare and analyze the short-term clinical efficacy between laparoscopic and open D 2 radical gastrectomy for those elderly patients with distal gastric cancer.Methods: The clinical data of 96 aged patients with distal gastric cancer,who received operative treatment from Mar.2009 to Mar.2013,were retrospectively analyzed.In these cases,38 cases were in laparoscopic group and 58 in open group.The lymph node dissection amount,operative time,blood loss,postoperative recovery,WBC,CRP,GPT and complications of two groups were compared.Results: No significant difference was found in the amount of resected lymph node and the average operative time between these two groups respectively(P 0.05).In the aspects of intraoperative blood loss,the recovery time of bowel function,postoperative hospital stay and the value of WBC,CRP and GPT on the third postoperative day,the laparoscopic group showed more distinctive advantage than the open group with the statistically significant difference(P 0.05).The rates of postoperative incision complications,lung infection and postoperative total complications in laparoscopic group were significantly fewer than those in open group(P 0.05).Conclusions: Laparoscopic D 2 radical gastrectomy has advantages of little blood loss,quick recovery of intestinal function,short hospital stay and mini-invasion.This procedure is safe and feasible for elderly patients with distal gastric cancer,which presents the superior character of minimal invasion and can achieve oncologic radical effect.
