Title: Treatment of temporal lobe epilepsy using transcorticai selective mygdalohippocampectomy
Abstract: Objective To observe the effectiveness and associated risks of trans-midtemporal trans- ventricle selective amygdalohippocampectomy via keyhole temporal craniotomy in treating mesial temporal lobe epilepsy. Methods 20 patients with intractable mesial temporal lobe epilepsy associated with unilateral hippocampal sclerosis were treated surgically using this surgical procedure and had been followed up more than 1 year. The outcome with respect to seizure control was evaluated according to Engel's outcome classification system. Results Class one outcome was obtained in 15 patients, class two outcome in 3 patients, class three outcome in 2 patients. No surgical mortality. No persistent neurological complications occurred. Conclusion In strictly selected patients with mesial temporal lobe epilepsy, this procedure is highly effective and safe.
