Title: MSCT MPR Diagnosis of Non-hiatus Diaphragmatic Hernia
Abstract: Objective To investigate the diagnostic value of non-hiatus diaphragmatic hernia(NHDH)using MSCT multiple planar reconstruction(MPR)method.Materials and Methods Image data of 23 cases with NHDH were analyzed retrospectively,of which Bochdalek hernia 8 cases,Morgagni hernia 3 cases(1 case confirmed by surgery),traumatic diaphragmatic hernia 9 cases(all confirmed by surgery),iatrogenic diaphragmatic hernia 3 cases.All the 23 cases underwent MSCT volume scanning,The MSCT axial and MPR images were reviewed by three experienced radiologists who were unaware of the results of surgery.The following signs were observed,including abnormally elevated hemidiaphragm,diaphragmatic discontinuity,visceral herniation,collar sign and dependent viscera sign.And then give the diagnosis separately.Results On axial and MPR imaging,the diaphragmatic hernia sac and contents,discontinuity of the diaphragm were observed in 22 cases of NHDH(13 cases displayed on the horizontal axis,and 22cases in MPR images,t=4.97,P0.05).2 cases of Bochdalek hernia mistaken for aging diaphragm,one misdiagnosed lipoma,one cases of large Morgagni hernia misdiagnosed lipoma,4 cases of traumatic diaphragmatic hernia were prompted the diagnosis in the first diagnosis,2 cases of iatrogenic diaphragmatic hernia missed,13 cases(56.5%)were diagnosed preoperated.14 cases(60.7%)were prompted diagnosis of NHDH on axial,21 cases(91.3%)on MPR by retrospectively analysis(t=4.33,P0.05).Conclusion MSCT MPR can clearly show the discontinuity of the diaphragm and the relationship between the diaphragm hernia sac and diaphragm in NHDH,and to determine the diagnosis of NHDH.
