Title: Clinical experience of complicated minimally invasive gallbladder-preserving operation
Abstract: Objective:To discuss the operation skills of complicated minimally invasive gallbladder-preserving operation.Methods:The retrospective analysis was made on clinical data of 56 cases complicated minimally invasive gallbladder-preserving operation from Jun.2008 to Jun.2010.There were 17 cases of adhesion around gallbladder,13 cases of cholecystic acute hyperemia and edema,9 cases of operation history of upper abdomen,8 cases of intrahepatic gallbladder,3 cases of calculus carcerated at cystic neck,2 cases of stones in cystic duct,2 cases of gallbladder diverticula,1 case of cholecyst which was full of calculi,and 1 case of cystic duct variation.Results:Fifty-four cases of endoscopic gallbladder-preserving operation were successfully performed and 2 cases were converted to open surgeries.One case of gallbladder could not be exposed because of compact adherence,and one case was caused by obstructed cystic duct.Conclusions:The principles of complicated minimally invasive gallbladder-preserving operation includes clear anatomy,sharp dissection close to the gallbladder and unobstructed cystic duct.
