Title: Synergistic effect of CsA and DBMC on heterotopic rat cardiac allograft survival
Abstract: Objective To summarize the experience of long-term survival of the patients under- going orthotopic heart transplantation.Methods After male inbred Lewis(RT1~1)rat heart had been transplanted into the abdomen of male inbred BN(RT1~n)rat,the experimental rats were divided into 4 groups:group A served as control,receiving 1 ml saline infusion into the portal vein intra-operation for 7 days after heart transplantation;group B received cyclosporine(CsA)5 mg/(kg·day)for 7 days after surgery;group C treated with CsA(5 mg·kg~(-1)·d~(-1))combined with 1×10~8 DBMC infusion in- tra-transplantation and group D treated with 1×10~8 DBMC for 7 days after surgery.The survival time of donor hearts was observed.The level of serum 1L-2 was detected by enzyme-linked immunosorbent assay(ELISA)and the mRNA expression of TNF-αin the graft by RT-PCR on 6th day following the operation respectively.HE staining was used to examine all specimens of heart grafts on 6th day after operation.The number of CD3~+ CD25~+ cells,antigen presenting cells(APC)from donor,the ratio of CD4~+ CD45RC~+/CD4~+ CD45RC~- and CD86~+ ceils in the peripheral blood lymphocytes of receptors were measured by flow cytometry on day 6,12 and 18 after surgery.Results The mean graft survival time in the groups A,B,C and D was 7.2±0.4,15.2±2.3,21.6±3.2 and 7.8±0.8 days,respec- tively.There was significant difference in the ratio of CD4~+ CD45RC~+/CD4~+ CD45RC~-,the expres- sion of CD86 on the APC,the expression of TNF-αmRNA,the severity of acute graft rejection and serum IL-2 concentrations 6 days after surgery between group C and other groups(P0.05).The typical features of acute rejection were most marked in group A and the least in group C.CD4~+ CD25~+ cells had no significant changes in these groups.There was no significant difference in number of APC derived from donors between groups C and D at 18th day after surgery,but it was more than that in groups A and B(P0.05).Conclusion DBMC infusion after transplantation combined with short- term treatment with CsA can induce donor specific low-reactive action and prolong the allograft survi- val in rat cardiac transplantation.
