Title: Strategy for prevention of HBV recurrence and reinfection in patients with hepatitis B related liver disease after liver transplantation.
Abstract: OBJECTIVE To explore the strategy for prevention of HBV recurrence and reinfection in patients with hepatitis B related liver disease after liver transplantation.METHODS The data of 79 patients with OLT because of hepatitis B related liver disease from Apr 2004 to Oct 2008 were analyzed retrospectively.According to the prophylactic strategy for HBV recurrence and reinfection, the 79 patients were divided into three groups, namely,A group(single lamivudine group),B group(combined medication of lamivudine and low-dose HBIg)and C group(combined medication of lamivudine and high-dose HBIg).The hepatitis B recurrence and reinfection rates were statistically analyzed.RESULTS The recurrence rate of A group was 33.3%,which was higher than the rate of B group(7.1%)and C group(5.6%).The rate of YMDD in A group was 80%,which was higher than the rate of B group(0)and C group(0).The rate of hepatitis B recurrence and reinfection in patient with positive HBeAg and HBVDNA of B group was 30%,which was higher than 4.35% of C group.CONCLUSIONS The combined lamivudine and HBIg are more effective than single lamivudine for protecting HBV recurrence and reinfection in patients with hepatitis B related liver disease after liver transplantation. The rate of hepatits B in patients with positive HBeAg and HBVDNA can be significantly decreased by using high-dose HBIg.
