Title: A clinical study on basket manipulation combined with ureteroscopy and holmium:YAG laser lithotripsy for treatment of proximal ureteral calculi
Abstract: Objective To evaluate the clinical efficacy of basket manipulation combined with ureteroscopy and holmium:YAG laser lithotripsy for treatment of proximal ureteral calculi.Methods 55 cases of proximal ureteral calculi who underwent ureteroscopic lithotripsy using holmium:YAG laser with a rigid ureteroscope.These cases were divided into two groups randomly:control group(25 cases) which didn't apply basket manipulation and observation group(30 cases) which applied basket manipulation.Results The successful rate of fragmentation in a single procedure for the proximal ureteral calculi was 89.1%(49/55).The calculi retrograded to renal pelvis or calix in 6 cases(5 cases in control group and 1case in observation group) who were treated by extracorporeal shock wave lithotripsy(ESWL) afterwards.There was higher difference between two groups(P0.01).No complications such as perforation occurred during the operation.The calculus-free rate was 100%(49/49).No ureteral stenosis occurred.Conclusions Basket manipulation combined with ureteroscopy and holmium:YAG laser lithotripsy for treatment of proximal ureteral calculi could reduce the rate of calculi retropulsion so that improving the successful rate of calculi fragmentation.
