Title: Clinical observation of postmenopausal bleeding caused by endometrium pathological change
Abstract: Object To investigate the reason of postmenopausal organic or the non organic endometrium bleeding, the relationship between carcinoma of endometrium and precarcinomatous pathological changes.Methods All patients received routine physical and gynecological examination,and took cervical scraping.The thickness of endometrium was measured by ultrasonic B,and the abdomen or vagina,as well as the female hormones including the follicle stimulating hormone(FSH),luteining hormone(LH),estrogen(E 2),progestogen(P),prolactin(PRL) were determined by radioimmunssay.Fractional curettage were executed and the thing which had scraped out was sent to be done pathologic histology examination.The statistics method adopted χ 2 test and t test.Results The endometrium pathologic examination showed that the proliferative phase of endometrium 69 cases,cystic hyperplasia of endometrium 18 cases,adenomatous hyperplasia of endometrium 6 cases,atrophic endometrium 16 cases,serious atypical hyperplasia of endometrium 4 cases,carcinoma of endometrium 2 cases.Secreting phase compared with the other pathologic histology types:P0.01;atrophic type compared with the other pathologic histology types:P0.01.The occurrence rate of postmenopause cystic hyperplasia or adenomatous hyperplasia causing carcinoma of endometrium was 8.3%.Conclusion The reproductive organ inflammation and the non organic factors were the major reasons causing the postmenopausal bleeding.The age is older,the fixed number of years for menopause is longer,and the bleeding which is apart from the first visit time is longer,the occurrence rate of malignant tumour is higher.Therefore the postmenopausal patients should be taken regularly gynecological examination as early as possible.The common disease causing postmenopausal bleeding should be cured actively and the occurrence of malignant tumour can be reduced.
