Title: Effect of Thoracic Epidural Blockade on Microcirculation in Pig Hepatic Ischemia-reperfusion
Abstract: Objective: To investigate the effect of thoracic epidural blockade on microcirculation in hepatic ischemia-reperfusion.Methods: Fourteen healthy domestic pigs were randomly divided into 2 groups(n=7): epidural blockade(group E) and control(group C).The animals were premedicated.Then the trachea was intubated and mechanical ventilation was provided to all animals.After induction of anaesthesia,the animals were placed in the right lateral position for insertion of an epidural catheter(T8-9).The left hepatic artery and left branch of portal vein were isolated via a midline incision.After 30 min,The group E was given 0.5% bupivacaine with 0.75 mL per segment to block T5~T12,the group C was given same volume saline.After 15 min,hepatic warm ischemia was produced by clamping of the left hepatic artery and left branch of portal vein of 45 minutes;this was followed by a 4 hours period of reperfusion.Aspartate aminotransferase(AST),Alanine aminotransferase(ALT),Nitric oxide(NO),Endothelin(ET-1),Tumor necrosis factor-α(TNF-α),Interleukin-1β(IL-1β) and Adenosine triphosphate(ATP) were determined before(T0) and after ischemia(T1),and at 1h(T2),2h(T3),4h(T4) after reperfusion.Liver was obtained to determine Malondialdehyde(MDA) and microscopic examination at the end of experiment.Results: In group E AST,ALT,ET-1,TNF-α,IL-1β,MDA were significantly decreased as compared with group C(P0.05);while NO and ATP were significantly increased(P0.05).Microscopic examination showed that liver damage was less severe in group E than in group C.Conclusion: Thoracic epidural blockade can increase the energy generation,improve the balance of NO/ET,reduce the generation of proinflammatory mediators,which can reduce the extent of hepatic ischemia-reperfusion injury.
